A. BUILDING ACCESS HOURS

B. ACCESS RIGHTS
Carnegie Mellon University in Qatar provides access badge to every new employee – staff & faculty, students, visitors and contractors. Each badge has different access rights.
New Issuance
Staff & Faculty
-
Complete the ID Badge Application Form duly signed by the Human Resources Department.
-
Submit the form to the Director of Facilities for approval/endorsement.
-
Submit the form to the FM Supervisor for ID badge issuance with appropriate access rights, information and photo.
-
Printed ID Badge should appear with the following contents: picture, name, position, CMU logo, ID badge number (back portion).
-
Once completed, applicant must sign at ‘Received by’ portion of the ID Badge Application Form.

Note: Upper color background is red (see sample badge here).
Students
-
Acquire ID Badge Application Form from Admission’s Department.
-
Completed application form should be approved/endorsed by the Admissions Director.
-
Submit the form to the Director of Facilities for approval/endorsement.
-
Submit the form to the FM Supervisor in order to get appointment date (per batch).
-
Upon appointment date, photo taking and ID badge issuance will take place.
-
Printed ID Badge should appear with the following contents: picture, name, batch year, CMU logo and ID badge number (back portion).
-
Once completed, applicant must sign at ‘Received by’ portion of the ID Badge Application Form.

Note: Upper color background is gray (see sample badge here).
Visitors
-
Complete the Visitor's ID Badge Application Form
-
The completed form must be signed by the requester’s Department Head.
-
Submit application form to the Director of Facilities for approval.
-
FM Supervisor will issue the ‘pre-printed’ badge containing the following: name as 'VISITOR’, CMU logo and ID Badge Number (back portion)
-
Requester will sign on the ‘Received By’ portion on behalf of the visitor.

Note: Pre-printed badge doesn’t require photo taking. Upper color background is red (see sample badge here).
Contractors
-
Official contractors should be endorsed officially by the requesting department's head.
-
Signed Service Agreement or signed Work Request should be presented to the Director of Facilities.
-
Contractor personnel will complete the Contractor's ID Badge Application Form.
-
Upon approval of the Director of Facilities, FM in-charge will create pre-printed badge with their appropriate access rights.
-
Contractor will sign on the ‘Received By’ portion of the ID Badge Application Form.

Note: Pre-printed badge doesn’t require photo taking. Upper color background is yellow (see sample badge here).
Replacement
Whenever the ID Badge holder losses his/her card, the following vital steps should be observed:

Report the ID badge loss to FMHotline via e-mail.
-
FM will deactivate the ID badge immediately.
-
Complete the ID Badge Replacement Form.
-
Completed form duly endorsed by the requester's Department Head should be sent back to FM.
-
FM will issue the new ID badge.
-
Note: As stated on the Request for ID Badge Replacement Form, applicable fees will apply as follows:
1st Loss Replacement Free of Charge
2nd Loss Replacement QAR25.00
3rd Loss & Up Replacement QAR50.00
Change of Access Rights
Upon request, new access rights will take effect immediately. The request may come from the ID Badge holder duly approved by his/her Department Head or it may come from the Department Head himself. Here are the following procedures, to wit:

Complete the Change of Access Rights Form.

Form should be approved by the requester’s Department Head.
-
Submit the completed form to FM Director for approval.
-
Upon activation of the new access rights, an e-mail of confirmation will be sent to the requester.

D. SECURITY AND SAFETY

Lost and Found Items
-
Exert all efforts to locate the lost item
-
Report the lost item to the reception guard
-
If item is found, present valid ID to the reception guard to claim
Fire Safety and Evacuation
DO NOT's
-
Use lift if the fire alarm goes off
-
Go back to fire site without the ‘clear or go signal’
DO’s

If you discover fire, close the door where the fire is located
-
Activate the nearest fire alarm button
-

FIRE ALARM sounds, leave the building by the nearest exit

Use the stairs

Assist people with disabilities

Proceed to Assembly Point 21 or Assembly Point 24 and report to Fire Warden

Wait for ‘go signal’ from safety staff to re-enter the building

E. QATAR FOUNDATION / EDUCATION CITY WIDE
Vehicle Access Tag (VAT)
Application Procedure:
-
Complete the Vehicle Access Tag Application Form
-
Attach copy of Qatar Residence ID
-
Submit to the office of Building Operations Supevisor
-
Collect the VAT within 2 days
Guidelines:
-
The Vehicle Access Tag is linked to the person who signed in for the VAT Application Form.
-
The VAT is not transferable from person to another.
-
The VAT may be assigned from one vehicle to another.
-
The VAT must be visible in your vehicle at all time while inside Education City, preferably hanging at your rear view mirror.
-
Failure to display the VAT may result to QF security towing your car out of the parking area and may result to turning over the vehicle to the Qatar Traffic Police.
-
The VAT must be surrendered upon termination of employment contract.
