Statement of Support

Each nomination must be accompanied by two statements of support (from someone other than the nominator) which describes how the nominee fulfills the qualifications of the selected category. Complete the form below for the nominee. All fields are required in order to process the statement of support. (Please do not submit more than two statements of support or one nomination form for each nominee.)
