New Students
The following information is required in accordance with university policy and must be submitted by incoming new students prior to the start of school. Without a completed medical file, they will not be able to register for classes and if applicable jeopardize residency privileges in campus housing. Housing assignment and visas of international students will be held until the information is received.

Complete Medical History Report. Making sure that the "For All Students" section at the end of the form is signed. This information will be used if the student becomes ill during their stay on campus and will help us provide care that meets individual needs of students. This information is confidential and will not be released without student consent.

The student must ensure that they receive all the appropriate vaccinations as outlined on the health form. This information helps us to protect the student and the community against vaccine-preventable diseases.
